What to include in a Medical Summary

There are so many layers to having a chronic illness, a huge part being the admin involved; having multiple medical appointments, different specialist teams and forever chasing referrals, repeat prescriptions – all whilst trying to manage symptoms and daily life. Safe to say it’s challenging at the best of times, which is why it’s very important to find ways to make our day to day life a little easier.
 
A medical summary is a document with key information on, such as your medical conditions, current medications and allergies. Even though our notes are mainly computerised, I have found it extremely helpful to make my own of which I can keep up to date and give any new doctors/consultants I may see. They can also be very helpful for emergencies or times where your unable to advocate for yourself, so you can simply hand over the medical information.
 

Things to include on your medical summary;

Personal details (name, date of birth, address, contact information, NHS number)
GP information (name, address, contact information)
Next of Kin (name, relationship to you, contact information)
Medical conditions/Medical devices
Current medications (medication name, dose and when you take this)
Allergies (specify reaction)
Other (any additional information you’d like to add)
 
There are companies which you can buy templates/booklets which you can simply fill in, or you can make your own!
